However, there are no implant design and related biomechanical investigations specifically for TCVO. Four variations (straight and contoured plate profile × convergent and divergent proximal screw direction) with two loading conditions (compressive loads and torsional load) were compared by finite-element method. Fracture risk and construct stability were chosen as the comparison indices. For both loading conditions, the fracture risk of screw, plate and bone was the lowest in straight plate with divergent screw direction (SD), while contoured plate with convergent screw direction (CC) was the highest. Similar results were found in construct stability, SD allowed the smallest micromotions of the L-shaped opening gap, but CC allowed the highest. Divergent screw direction can decrease fracture risk of all components and provide better construct stability, while contoured plate profile seems like to have converse effects. If stability is the major concern, straight plate with divergent screw is recommended for patients with heavy load demands. Contoured plate may be suitable for skinny patients that can reduce the soft tissue irritation.Background Charcot-Marie-Tooth (CMT) disease is a hereditary motor-sensory neuropathy that is often associated with a cavovarus foot deformity. Limited evidence exists for the orthopedic management of these patients. Methods Thirteen experienced, board-certified orthopedic foot and ankle surgeons and a neurologist specializing in CMT disease convened at a 1-day meeting. The group discussed clinical and surgical considerations based upon existing literature and individual experience. After extensive debate, conclusion statements were deemed "consensus" if 85% of the group were in agreement and "unanimous" if 100% were in support. Conclusions The group defined consensus terminology, agreed upon standardized templates for history and physical examination, and recommended a comprehensive approach to surgery. Early in the course of the disease, an orthopedic foot and ankle surgeon should be part of the care team. Level of evidence Level V, expert opinion.Objective Multisource longitudinal network analysis was used to determine if between-child and within-child variance of attention-deficit/hyperactivity disorder (ADHD) symptoms provided unique findings of ADHD relative to latent variable model (LVM) analyses. Method Mothers and fathers of 802 Spanish first-grade children (54% boys) provided ratings of ADHD symptoms at two time points six weeks apart (assessment 1 723 mothers and 603 fathers; assessment 2 667 mothers and 584 fathers). Network and latent variable models were applied to the ratings. Results Inattention, hyperactivity, and mixed hyperactive/impulsive symptom communities occurred for the within- and between-children's symptom networks with the results being consistent across mothers and fathers, especially for the between-children's symptom networks. LVM analyses identified three factors with the same symptoms on each factor as in the symptom communities. These models also showed invariance across mothers and fathers as well as assessments. Conclusions Longitudinal networks provided several useful insights for ADHD, including centrality symptoms that differed across between- and within-child levels. However, many findings were also largely consistent with the LVM analyses.
